This school year the VCS athletic department will continue implementing the use of FinalForms. This platform will be utilized for completion of athletic participation forms as well as timely communication regarding attendance and injury updates. This will be the platform for submission of student physicals moving forward. 

Please create or update your FinalForms account prior to your first sport tryout. Registration can be completed here following the prompts. We are asking that student accounts utilize their warrior life email. An account is still necessary even if a paper physical has been submitted to the athletic office. If this is the case, please email sportsmedicine@vcs.net.
